Post Map Tool implementation, the proportion of patients with LDL < 100 who had lipid control increased 9.2 percentage points in the general population and 9.6 percentage points in the Medicare population. Post Map Tool implementation, the proportion of patients with HbA1c < 7 who had glycemic control increased 8.7 percentage points in the general population and 18.3 percentage points in Medicare population; Diabetes education had a significant effect (p = 0.02) on patients with a baseline HbA1c <= 9% and those with poor control.
