Today Healthy Interactions announced their expanded partnership with Village MD to deploy patient education programs for additional chronic illness. This expansion builds on the success of the existing diabetes program, which will now be offered at several more locations.
In 2015, Village MD chose Healthy Interactions’ Conversation Map® tools for diabetes self-management education (DSME) to improve education and engagement among their diabetes patient population. Over the past year, Village MD patients with diabetes who have gone through the Healthy Interactions’ program have shown significant improvement in their health outcomes (including HbA1c) and satisfaction. Due to these results, Village MD has decided to expand their current program to include Conversation Map-based education programs for COPD, CHF, and DPP and expand from 1 to 4 Village MD health centers in the Houston, TX area. These additional locations will also offer the DSME program in Spanish to meet the needs of even more patients.
The expanded partnership will also include Healthy Interactions’ Digital Engagement Platform (DEP). The DEP connects patients to Village MD’s care team members for sharing of health information, goals, and educational resources and for care management coaching. The DEP is one of the only known care management platforms that has achieved HITRUST CSF Certified status, ensuring that patient health information stays protected, providing peace of mind for health systems.
