This partnership marks a significant milestone in the effort to transform primary care delivery through innovative, evidence-based strategies. By joining the Healthy Interactions Primary Care Alliance, the NNE CO-OP PCBRN will gain access to scalable training programs, digital tools, and peer-to-peer learning platforms designed to support sustainable practice transformation.
The collaboration comes at a time of growing concern over cardiometabolic health in northern New England. The region faces a high burden of cardiometabolic conditions—including obesity, diabetes, hypertension, and cardiovascular disease— which is compounded by its older population, socioeconomic barriers, rural geography, and workforce shortages. These challenges underscore the urgent need for innovative, community-driven solutions that bridge the gap between research and practice.
