JACKSON HOLE, Wyo., -- Healthy Interactions LLC, has announced a strategic new partnership with Clinical Directors Network (CDN). As a global leader in experiential education for healthcare professionals and patients, Healthy Interactions plans to collaborate with CDN and significantly advance the transformation of primary care delivery, with a particular focus on improving cardiometabolic health outcomes across diverse populations.
CDN is the newest member to join the Healthy Interactions Primary Care Alliance—a national consortium of primary care clinics committed to evidence-based practice transformation and community centered care. With a strong presence in New York and a national footprint as an Agency for Healthcare Research and Quality (AHRQ) designated Center of Excellence for Practice-based Research and Learning, CDN brings extensive experience in practice-based research and implementation science.
The integration of trained primary care educators into clinical workflows will enhance patient education and engagement. CDN’s expertise in online continuing medical education and practice facilitation and supporting community health initiatives will be instrumental in driving sustainable improvements in cardiometabolic care.
