Baseline (usual care group): No difference in knowledge, self-care activities, or empowerment baseline and post-survey for usual care group.
Conversation Map® Group: significant improvement in knowledge, self-care activities, and empowerment when comparing baseline and post-intervention surveys for Map Tools/intervention group; no difference in satisfaction between usual care and treatment groups; HbA1C results improved for both groups; average A1c reduction for both groups was 6.7% (p-value = 0.009).
