Healthy Interactions turnkey Integrated Chronic Disease education deployed at Pottstown Medical Specialists Inc. were shown to substantially help patients with COPD.These educational Conversation Map sessions included a series of clinically-validated, reimbursable in-person sessions. Healthy Interactions COPD Conversation Map program delivers the highest standards in COPD self-management education and patient empowerment. COPD self-management education is an essential part of achieving better outcomes in COPD disease management. While COPD education is typically limited to rehab, this unique methodology is found to be highly successful in primary care practices.
Healthy Interactions COPD Conversation Map tools are now used by a network of diabetes educators who have been using the unique methodology of diabetes education for the last decade. With an educator network of over 30,000 nurses, dietitians, pharmacists and doctors can use the trusted methodology for other medical conditions. In this case, the Healthy Interactions sessions can be used in primary health systems as part of COPD self-management education. Pottstown Medical Specialists, Inc. was formed in August of 1978, first known as Deviney, Guthrie & Popolow, LTD, from the merging of three separate practices of internal medicine. In May of 1980 the company name became Pottstown Medical Specialists, Inc. (PMSI).
